46 year old female, with prior history of external nasal trauma multiple times who was unhappy with the appearance of her nose and had problems breathing through her nose due to a deviated nasal septum. She wished to have a septoplasty to improve her breathing and a cosmetic rhinoplasty to address the issues of her nose which bothered her the most. She wished a reduction in her nasal bump, a reduction of the nasal tip width, improved tip definition and elevation of the nasal tip. She underwent a septoplasty and open rhinoplasty. The rhinoplasty lowered the nasal dorsum to remove the external appearance of a bump. The tip cartilages were non-destructively shaped and approximated with sutures to decrease the tips width and refine its definition and rotate the tip upward to create a subtle supratip break. A small sliver of the caudal septum was removed to help with tip rotation. Her turbinates were shrunk with bipolar cautery. Her post operative photos were taken 6 weeks after her rhinoplasty/septoplasty and show a straighter, smooth dorsum with a refined, narrower tip which has a upward rotation and a subtle supratip break. She is very pleased with her new nasal appearance and improved breathing.
